Medication forms

Any new medication brought to school must be signed in with a Medication form. This applies to both Prescribed Medication from your child’s Doctor/Paediatrician and non-prescribed medications such as Calpol, Cough Medicine, Ibuprofen etc.

Prescribed Medication must have your child’s name on and must be in the original packaging you received from the Pharmacy.
